MANCHESTER 09/11/2014 – Minuteman Health (http://www.minutemanhealth.org) today announced that the Littleton-based North Country Accountable Care Organization (http://www.northcountryaco.org) has joined its provider network. “North Country’s partner treatment centers provide superb care to Coos and Grafton county residents, and their participation in Minuteman’s network gives us a footprint in nine of the state’s ten counties,” said Minuteman chief medical officer Jan Cook. “We’re very pleased to have the opportunity to work with them in bringing a new approach to healthcare.” North Country ACO consists of four community health partners (Ammonoosuc Community Health Center, Coos County Family Health Services, Indian Stream Health Center, and Mid-State Health Center) with seven clinic locations. North Country is dedicated to promoting better care coordination and patient outcomes while promoting a collaborative environment across healthcare systems in the state’s rural north counties.
