As a MinuteMember, getting your annual preventive visit with your In-Plan Primary Care Provider (PCP) will pay off in more ways than one this year. Not only will your health benefit—your wallet will too.

HERE’S THE DEAL:

Minuteman Health members who are enrolled, fully insured and 18 years and older will receive a $50 incentive check after they complete their annual preventive visit with their In-Plan Primary Care Provider any time between January 1, 2017, and December 31, 2017.

Members can expect to receive their incentive check during the quarter following their preventive visit. For example, if you had your preventive visit by June 30, 2017, you can expect to receive your incentive award in September.

There’s even more good news: There is no paperwork to fill out. We’ll do all the work by identifying the annual preventive visit bill we receive from your In-Plan PCP. That sounds easy, right? So, if you haven’t already, schedule your annual preventive visit now.

Q: Who is eligible?
Members who have a preventive visit any time during the 2017 calendar year, are enrolled and 18 years of age or older at the time the PCP visit occurs are eligible to receive the incentive payment. HMO members must use an In-Plan PCP. PPO members can see an In-Plan or an out-of-plan PCP.


Q: Is the incentive limited to the preventive visit or are other preventive services included?
The incentive is limited to the preventive visit recognized by the preventive visit codes submitted by the provider at the time of the visit. The preventive Evaluation and Management codes that qualify are: 99385, 99386, 99387, 99395, 99396, 99397. Preventive services such as Mammograms and other routine screenings are not included in this program.

Q: What do I need to do to get the incentive reward?
MHI members simply need to be enrolled and have an annual preventive visit (see above codes) with an In-Plan PCP (PPO members can also choose from out-of-plan PCPs).

Q: What is the incentive amount and how will it be issued?
Qualifying Members will receive a check issued by MHI for $50. The checks will be mailed to the member mailing address that we have on file.

Q: When can members expect to receive the incentive reward?
MHI will issue and mail wellness reward payments to Qualifying Members on a quarterly basis. For example, if you had your preventive visit by June 30, 2017, you can expect to receive your incentive award in September.

Q: Are there limits to families or individual members?
All adult (18 and over) members of a family are eligible to earn the incentive. Individual members are limited to one $50 incentive reward per year.

Q: What if I had my preventive visit and haven’t received my incentive reward?
The date that a claim is submitted by the PCP and the member’s enrollment status can impact the date that a paid claim appears in the MHI claims system. This is especially true for members who have a preventive visit during the last weeks of the quarter. An incomplete or wrong address can also delay or prevent the incentive from being delivered.
